Tenant Work Order Request
Effective January 1, 2003, due to State Laws, all maintenance requests must be submitted in writing in order for Citywide to provide service. All emergencies must be reported by telephone and followed up with a written request. Please complete and submit the following work request form.
* If the answer is no, please note that tenant will be responsible to provide the contractor with access to the unit which may require a three hour window for appointments. Any missed appointments made by the tenant that result in fees will be the responsibility of the tenant.
** Please let Citywide or the contractor know if there are any animals in the home or special instructions that we need to be aware of.
